Occupational Therapist (Permanent Supportive Housing – PSH)

Location: San Fernando / Antelope Valley, CA 
Schedule: Day Shift (8:00 AM – 4:30 PM)
Compensation: $55.00 per hour
Position Type: Field-Based, Short-Term / Registry Coverage

Overview

The Licensed Occupational Therapist (OT) provides short-term, field-based occupational therapy services to individuals residing in Permanent Supportive Housing (PSH) who have experienced homelessness and present with complex functional, cognitive, or behavioral health needs.
Under the supervision of an Occupational Therapist Supervisor or Occupational Therapist II , the OT conducts functional assessments and delivers client-centered interventions to promote independent living, tenancy stabilization, and improved quality of life.

This position is designed to provide flexible clinical coverage across multiple PSH programs within Los Angeles County Department of Health Services (LAC DHS) – Housing for Health (HFH) initiatives. Assignments may vary in length and location depending on operational needs, including coverage for staff vacancies, program surges, or project-based deployments.

About the Program – Housing for Health (HFH) & Permanent Supportive Housing (PSH)

Housing for Health (HFH) is a division within LAC DHS dedicated to reducing homelessness by integrating health services and housing support.
Permanent Supportive Housing (PSH) combines long-term housing with wraparound services for individuals with chronic health, mental health, or substance use conditions. Clients are supported by Intensive Case Management Services (ICMS) teams that promote housing stability, health access, and engagement in meaningful daily activities.

Occupational Therapists play a critical role in PSH by addressing the functional, cognitive, and environmental barriers that impact clients’ ability to maintain housing and live independently.

Key Responsibilities
  • Conduct functional evaluations under supervision to assess ADL/IADL performance , functional cognition, and life management skills.

  • Design and implement individualized interventions that support transitions into housing, routine development, and self-management.

  • Participate in interdisciplinary case conferencing to ensure coordinated and holistic service delivery.

  • Recommend adaptive equipment, environmental modifications , or supports to enhance independence and safety.

  • Document services , progress, and recommendations in compliance with program and departmental standards.

  • Provide field-based clinical services in client homes, community settings, and supportive housing environments.

  • Supervise OT students or aides as assigned.

  • Engage in program improvement activities (e.g., pilot projects, trainings, client feedback) under the guidance of OT leadership.

Desirable Qualifications

Experience & Expertise

  • Experience (including fieldwork or early career) providing occupational therapy to individuals experiencing or at risk of homelessness, ideally in community-based or public health settings .

  • Familiarity with complex care coordination , functional cognition, mental illness, substance use, and physical disabilities.

  • Knowledge of public and social service systems , including disability benefits and housing programs.

  • Ability to work effectively with clients from diverse cultural, socioeconomic, and linguistic backgrounds .

  • Bilingual proficiency (especially in Spanish ) is preferred but not required.

Clinical & Technical Skills

  • Understanding of trauma-informed care , functional assessment tools , and activity-based interventions in non-traditional environments.

  • Adaptability in delivering interventions across a wide range of physical, behavioral, and cognitive needs.

  • Comfort working in field-based, community settings .

Interpersonal & Team Collaboration

  • Strong communication, empathy, and relationship-building skills.

  • Proven ability to work collaboratively within multidisciplinary teams (e.g., case managers, nurses, housing providers).

  • Receptive to supervision, clinical feedback, and ongoing professional development.

Minimum Requirements
  • Current California Occupational Therapist License (OTR/L).

  • Valid Driver’s License and reliable transportation for field-based work.

  • Clearance of all required background checks and health screenings per LAC DHS policy.

Compensation & Schedule
  • Pay Rate: $55.00 per hour

  • Schedule: Day shift, Monday – Friday (8:00 AM – 4:30 PM)

  • Work Type: Field-based, short-term/registry coverage across PSH programs
